Paid for 20 year warranty but never got the paperwork

I had a service technician from All Seasons Garage Door Repair come out on July 24th to fix a broken spring. They did the work and I paid the invoice right then. The invoice had a premium charge on it for a 20 year warranty. The tech told me I would get a copy of the warranty with the terms and conditions in the mail or something.

Well, I've called the technician a few times since then asking about it. He keeps saying he'll send it but nothing ever shows up. Now I'm starting to think this company might not even be real. I can't find a physical address anywhere, and the phone number I have just doesn't seem to work right. No email either. I paid extra for that warranty and I have nothing to show for it. I'm wondering if anyone can look into this company for me.
