Invoice with a form asking for my business info

I got this invoice in the mail for $136.22. It came with a form called CF1003 "Corporate Existence and Transparency Reporting" and it was asking for all this info about me and my business.

The return address on it is 11199 Rte. 29 Ste. C #524, Fairfax, VA 22030. It just didn't make sense to me. I never ordered anything or signed up for any reporting service. It felt really strange to get a bill like this out of nowhere.

I didn't pay it, but I'm annoyed. They make it look official and like you have to do it. I just wanted to put this out there so other people know.
