Email about a free promotion through my work insurance

I got an email about a promotion that was supposedly through my employer-paid insurance agency. I called my insurance agency to check and they said the company offering it was legitimate. So I clicked the link and read the terms and conditions, all of it, I always do.

It said the program was completely free but they needed a credit card just in case I wanted to join other programs. I entered my personal info, health conditions, credit card, and insurance information. Then I got a notice saying I was approved. Right after that my card got hit with a $2.00 charge from CHARGEY. I never authorized that.

Thank God my bank declined it and let me know. I froze my card and now I'm stuck wondering who stole my identity. I told HR and they said they didn't have to enter that information, so somewhere between that link there was a data breach that sent me to a scam website.
