Form CF1003 asking for money and threatening fines

I got a form labeled CF1003 that said it was from Corporate Filings. They wanted a fee paid by check or money order for something called Corporate Existence and Transparency Reporting. There was also a threat that I could be fined up to $10,000 if I didn't send it in on time.

There was no return address anywhere on it, which seemed odd. I looked up the name online and found the real Corporate Filings LLC. I wrote to them and they got back to me by mail saying this was a scam. They told me they don't ask for that kind of information or those fees. I lost $136.22 to this.
