Letter with a check for $2,950 to do a survey

My husband got a USPS Priority Mail flat envelope. The label is in the photo I uploaded. Inside was a letter with a check for $2,950.00 telling him to do a customer service evaluation tool, CSET. The email on it is [email protected] and the address is 27959 Smyth Dr #100, Valencia, CA 91355. There's also a phone number, 1(347) 680-8222. I have a photo of the letter too but I could only upload one file.
