Job offer on Teams asked for my Social Security and license

I was hired through email and chat on Microsoft Teams. It was for a Telehealth RN job working from home, $35 an hour, which seemed normal compared to other jobs out there. The company was Enablihealth at their website. I found the contact on an online job hunting site called Sophie.hayes.lensa.com.

They sent me a contract and asked for my direct deposit info, a copy of my W2 with my social security number, and a copy of my driver's license. After I signed the contract everything looked legitimate. Then they wanted me to buy all this equipment for a home office, said they would order from a vendor and send me a check to deposit that would clear around $4,000. That's when I knew it was a scam.

Now they have my social security number and driver's license and I'm really concerned. I already froze my credit. I changed the routing at the bank and my bank account. I'm not sure what to do about my driver's license and social security.
