Greendot card in my name from a credit repair place

I was dealing with Gems Financial Services about fixing my credit. The lady there took my income tax money. Next thing I know there's a Greendot card in my name and purchases and withdrawals were made on it. I didn't do any of that. Her number is (214) 772-4414 and the email is [email protected]. The address is 8035 E R L Thornton Frwy, Dallas, TX 75228. I never got my tax money back.
