Letter about a waiver with no company name on it

I got this letter in the mail and there’s no business name anywhere on it. It just says Lender: Indiana Housing & Community Authority. They had my property code wrong too, which was weird. There was a number to call, 1-888-679-0672, and a $199.00 Allocated Waiver attached to it.

I didn’t call because something felt off. The letter didn’t explain anything, just gave that number and the waiver. I don’t know what it’s for or why I got it. The wrong property code made me think it wasn’t even meant for me.

I’m just confused. No company name, no real explanation, just a number and a fee. Didn’t lose any money, but I wanted to report it.
