Email about a promotion through my employer insurance

I got an email about a promotion that was through my employer-paid insurance agency. I checked with my insurance agency and they said the company offering the promotion was legitimate. So I clicked the link and read the terms and conditions, all of it, I always do. It said the program was completely free but a credit card was required just in case I wanted to do additional programs.

So I entered my personal info, health conditions, credit card, and insurance information. Then I got a notice that I was approved. Instantly my credit card was hit with $39.99 for SLUMBRAPP. I did not authorize that charge. Thank god my bank declined it and let me know. I froze my card and now I have to wonder who stole my identity. I told HR and they said they didn't have to enter that information, so somewhere between the link there was a data breach that sent me to a scam website.
