Email about a promotion through my work insurance

I got an email about a promotion that was supposed to be through my employer-paid insurance agency. I checked with my insurance agency first and they said the company offering it was legitimate. So I clicked the link and read the terms and conditions, I always read all of it. It said the program was completely free but they needed a credit card just in case I wanted to join other programs later.

I put in my personal info, health conditions, credit card, and insurance information. Then I got a notice saying I was approved. Right after that my credit card got hit with $39.99 for SAMADIAPP. I never authorized that charge. My bank declined it and let me know, thank god. I froze my card and now I have to wonder who stole my identity.

I told HR about it and they said they didn't have to enter that information, so somewhere between the link there was a data breach that sent me to a scam website.
