Acceptance form asking for SIN and ID before selling shares

We got a letter with a form called Acceptance Form. It says we have stocks with them and we can sell our shares to them. To do it we had to get the form notarized by a lawyer and send it with our SIN number and a picture of ID.

We did all that but when we went to mail it the postmaster said the postal code didn't work. So we didn't send it and started looking into it.

The phone number on it is 1(212) 523-0973 and the email is [email protected]. The address is 157 Adelaide street west suite 507, Toronto, ON M5H 4E7, Canada. Just trying to find out if this is a scam or real.
