Survey for CPR kit turned into a membership charge

I did this survey online to get a C.P.R emergency kit. The survey never said anything about joining a membership. Nowhere did it say that. But the fine print did clarify you had to cancel a membership to not get charged. Who reads fine print on a survey for a free kit?

I got charged $73.80 to my card. I didn't sign up for anything. I just wanted the kit. I tried to email them at [email protected] but haven't heard anything back. The phone number is (888) 314-8582 but I'm not sure it's even worth calling. It just feels like a way to steal my info. The address they have listed is 1425 Broadway PMS 482, Seattle, WA 98122. I don't know what else to do about it.
