Final notice letter with a $199 check that looks fake

I got a letter in the mail with U.S. Postage Paid, Permit No.1535 on it. The front said IMMEDIATE RESPONSE TO THIS NOTICE REQUESTED. When I opened it up it said FINAL NOTICE at the top.

At the bottom there was this blue check looking thing that said Allocated Waiver with $199.00 on it. It had my name, my address, and lender information on it too.

The phone number on it was (800) 227-1254 and the address was Boca Raton, FL 33431. I didn't lose any money but this whole thing just seems off.
