Monthly charge for birth certificate service I never signed up for

I needed a birth certificate for my son and they handled it. That was all I needed from them. Then I see a charge on my account for 9.99 monthly fee from USA-VREXPRS. I didn't know anything about this and was not advised of any recurring fee.

I don't need recurring services. I already completed my task with them. I called my bank and told them I did not authorize this payment, so now I have to get a new debit card. When I went online to see who USA-VREXPRS is, I saw other people went through the same thing with this company. Their phone number is 1(941) 264-6901 and they have a website at usarecordforms.com.
