Got a medical bill for services I never received

I got a medical bill in the mail from Captivate Billing. It's telling me I need to pay for some medical services. The problem is, I never received any services from them. I don't even recognize what they're billing me for.

The bill has their phone number as (720) 900-0101 and an email, [email protected]. Their address is listed as 2919 17th Ave Ste 217 in Longmont, CO. They also referenced a website, patientnotebook.com/cvfdhouston. None of this makes sense to me because I never went to any doctor or facility connected to this.

I shouldn't have to pay for something I never got. It just feels off getting a bill out of nowhere like this.
