Explanation of benefits for medical supplies I never got

On 05/27/2026 I got an explanation of benefits from Care First. It said there was a $5,047.60 claim on my insurance for medical supplies. I never got any medical supplies.

I looked into it and found out this company is known for sending fake claims. The phone number on it was (800) 695-5474 and the address was in Memphis, TN 38117. I don't know how they got my info but it feels strange. No one called me or sent anything before this.
