Loan company emailed a form asking for my bank account numbers

I was applying for business loans and then I got their phone number. They called me and then emailed me a form to fill out. The form already had all my info and business info on it like my EIN and bank state account numbers. That felt really strange.

I didn't send them any money or anything. But I'm worried about what they have. Their website is catalystcapitalconnect.com and their email is [email protected]. I just wanted to put this out there so people know.
