Got a weird letter about a data breach and identity monitoring

I got this letter in the mail talking about an alleged data breach from my "current or former health plan." It's offering 12 months of identity monitoring through Epiq Privacy Solutions, which is weird because I thought the standard was 24 months.

To enroll, they want me to use an activation code from the letter and also give my Social Security Number. The whole thing just seems super generic and fishy to me. The website is privacysolutionsid.com and the contact email is [email protected]. They also list a phone number: (877) 332-1658.

It just doesn't feel right. I didn't lose any money, but the letter itself and what they're asking for sets off alarms.
