Someone used my info to order something I never bought

I got a call from (216) 412-4822 saying I had an order from falconmed.com.br. I never ordered anything from them. They had my patient information somehow and used it to represent this false company. I told them I didn't send any order but they kept insisting.

I don't know how they got my info but it feels like identity theft. I didn't lose any money because I caught it but still. They're using patient data to pretend to be a real company. The website is https://falconmed.com.br/ and the phone number keeps calling me. I just want this stopped.
