Got a fax from my office about a $25 charge I never agreed to

I got a call from someone saying they were with Family Practice. They said I owed $25. I didn't remember this charge, but I paid it because they called me directly.

Then a fax showed up at my actual doctor's office for them to sign off on. That's when I knew something was wrong. I never authorized any of this. The fax came from a number (727) 201-1890 and an address in Saint Petersburg, Florida.

Now I'm out $25 and this whole thing feels off. The fax being sent to my doctor's office is what really didn't make sense.
