Mail about car warranty with exact year and model

I got this piece of mail and the header says "FinalNotice" in big letters. It had my exact car year and model right there in the topic. It was saying if my factory warranty expired I would be responsible for paying any repairs myself.

But then it said I could still activate a service contract on my vehicle before it's too late. There was a table at the bottom with terms and vehicle protection info. No company name on the letter though, just a phone number (888) 646-0339. Felt strange.
