Unsolicited VISA debit card showed up in the mail from Health Equity

I got a VISA debit card in the mail from a company called Health Equity, or someone pretending to be them. I never signed up for this and never even heard of them before. I have no idea if the card is real or fake.

I looked up their website, healthequity.com, and it seems like a real company with a customer service number. But when I tried calling, they asked for my date of birth and zip code just to "find your account." That felt off to me, especially since I never had an account with them in the first place.
