Got a weird letter about my mortgage with a strange phone number

I got this letter in the mail that says "Final Notice" about my home address and my lender, which is J P Morgan Chase. It says it's secured by my current lender. There's something about a $199.00 check attached but then it says right on it “this is not a check”.

The letter also says “we reserve the right to revoke allocated waiver within 7 days of receiving this notice.” It just feels off. It names my real lender, but the phone number on it is 888-801-8190. That's not their number. I don't know what this is, but it doesn't seem right coming in the mail like this.
