Letter about losing car coverage with no company name

I keep getting these letters ever since I bought my vehicle. This one says if I don’t respond I’ll lose coverage on my car. No business name anywhere on it. Just a guy named Jeff Morton, Program Coordinator, and it says Approval Division.

I don’t even know who these people are. How are they able to get your info like this. The phone number on the letter is (888) 926-1464. It just feels wrong.
