Phone call saying I broke a signed agreement with no company name

Got a call from someone named Kayla Sanders. She said I had a signed agreement that I didn't hold up my end of, and that they tried reaching me before but I never answered. She said since I didn't respond it got sent to their pre-legal department for review and a decision would be made without my statement.

She left a number to call her back at 470-508-4491 and said it twice. The caller ID showed 304-223-0903 which I looked up on Google and it came up as a mental health services place in Charleston WV. That doesn't make any sense. I never signed any agreement with anyone named Kayla Sanders or that phone number. No company name was given.
