Job offer from Materna Medical with no real interview

I got an email from them saying to fill out a questionnaire by 5/11 so I did. Then a couple days later I got another email back from someone named Sonia Garcia saying congratulations and there was a paper to sign.

It just felt off. They didn't really interview me or anything just that questionnaire and then suddenly I'm supposed to sign something. The email address was [email protected] and their address says 2495 Hospital Drive Suite 300 Mountain View CA. I didn't send any money or anything but it seems like some kind of employment scam.
