Billed my insurance for medical supplies I never ordered

I got a letter from my insurance saying Memphis DME LLC billed them for some supplies. I never ordered anything from them. Never even heard of them before this.

Their phone number is (800) 695-5474 and they have an address at 5118 Park Ave Ste 323 in Memphis TN. They just put in a claim to my health insurance for stuff I didn't ask for and never received. No idea how they got my info. It's just wrong they can do that.

Explanation of benefits for equipment I never got from a company I don't know

I got an explanation of benefits from CareFirst saying Memphis DME LLC billed them 4,910.40 for medical equipment. I never got any equipment. I dont even have CareFirst blue Choice anymore I switched to Medicare about a year ago.

The charge was from May 27th or 26th I dont remember exactly. But I didnt have CareFirst on that date because I already had Medicare. I have no idea who this company is or why they billed them. Never heard of Memphis DME LLC before this letter showed up.
