Billed my old insurance for medical supplies I never got

So this company Memphis DME SUPPLY billed my old insurance United Healthcare for a bunch of medical supplies I never ordered or received. They used codes like A4297KX and A6196A2 and a few others, with billing dates from January through May 2026. All of them say place of service was my home.

The thing is I haven't been with United Healthcare since 2023. I have no idea how they got my old member ID number. And what really gets me is they somehow connected it to my current address which was never on file with UHC. The insurance company denied the claims because I wasn't covered but that doesn't change the fact these people had my info.

Their address is 5118 Park Ave, Ste 323 in Memphis and their email is [email protected]. They called from (901) 443-1196. I just don't understand how they got all this information.

EOB showed charges from a company I never heard of

I got an EOB from my insurance and there was charges on it from some company I don't know. I called my insurance right away to dispute it. The company is called Memphis DME Supply and they got my medical info somehow and put in charges using my name. I never ordered anything from them or talked to them.
