Health insurance denial letter for medical supplies I never ordered

I got a claim denial letter from my health insurance company about Memphis DME Supply LLC. They tried to bill them $4068 for 3 months of medical supplies from January to March 2026. I never ordered or received anything from them. Never even heard of this company before.

I don't have any illness or injury that would need durable medical equipment. So it was weird. Luckily my insurance denied it because there was no medical evidence. So no money got paid out to them.

I think my health info got taken from a big data breach with ******* back in October 2025. I only found out about that a few weeks ago when MedStar told me.
