Blender showed up in my maiden name with no invoice

I got a package in the mail under my maiden last name from Montgomery Ward. It was a blender and I never ordered anything. I thought maybe it was a gift from someone but there was no information at all, no invoice or anything.

Then about a week later I got a bill in the mail asking me to make a payment. There were no return instructions and I can't find an address or website clearly on the package. I don't know what to do with this thing.
