Got a weird package and letter about a waiver I never asked for

Today I got a package delivered to my door from permit number 1535. I didn't open the package but I opened the letter inside. It was from Motor Vehicle Service. There was an allocated waiver attached to it for $199.00, saying the waiver could be revoked in 7 days.

I called the 1800 number and told them to stop harassing me. Get to the point. They said my down payment was $295.00, then $260.00 a month for 5 years. I told them to get bent and hung up. I also texted them, can't repeat what I said. I told them my medical bills take first dibs.
