Got a strange letter telling me to call about a waiver fee

I got this letter in the mail telling me to call 1-888-801-8190 immediately. It had a $199.99 waiver attached to it. The letter says Mr. Cooper, or Nationstar, is the lender. But here's the thing—my home is completely paid off. There's no other company information anywhere on the letter. It just says to call that number right away. The whole thing felt off.
