Paid for passport renewal and got nothing but a blank form

I was trying to renew my passport and found this site. It took my personal info and the payment of $125.90 to process everything. The US Passport service has no record of anything and I had to pay them again to get my renewal done. My Passport Center doesn't list a phone number or an address on their site. They just talk about their "commitment" to help with processing documents. After a whole month of waiting all I got was a DS-82 form that was completely blank. No directions or anything else.
