Weird job application asked for my passport and had the wrong job title

I got this document from Neshcgrp for a job application. It's just odd. The form had an upload for a driver's license and a passport. My passport is expired. I haven't traveled outside the country in years, and I've never been asked to provide my passport for a job.

Also, the job title on it was completely incorrect. I was interviewing for a remote financial clerk position, not a medical scheduler. This document would have everything needed to completely steal someone's identity. It just felt off.
