Sold my shares after filling out a notarized form but never got paid

I was offered to sell shares of my Principal Financial Group, Inc. insurance. So I filled out the form and got it notarized like they said. Did all that and now my shares are gone but I never received the 5480.20 dollars they told me I would get.

I have called and emailed them. No answer on email and the phone mailbox is full and not accepting calls. The address they gave me was 295 Madison Ave 12th Floor, New York, NY 10017. Phone number is 1(212) 523-0973 and email is [email protected]. I just want my money.
