Someone is using my NPI to pretend to be a healthcare worker

I found out someone is pretending to be me online. They are using my name and my NPI number to act like they are a healthcare professional. The website is serinaphillip.dr-leonardo.com and they have a Norfolk, VA address listed.

I have no idea how they got my information. I didn't lose any money but this is scary because they are using my professional license. I don't know who to report this to. It just feels really wrong that someone is out there pretending to be me.
