Someone changed my last name on my Social Security disability

I got my Social Security disability messed with. Someone changed my last name on my benefits. They used the email [email protected] and a phone number (303) 525-2233. The address they had was 1443 Oneida St 128 in Denver CO 80220.

So now my old last name is denied for benefits but the changed one they put in is approved. I lost $5000 from this. I don't know how they got into my stuff but they did. I didn't give anyone permission to change my name or my benefits like that.
