Order delivered to wrong address and they want a signed form

I ordered something online from Stylevana and paid for shipping insurance. It was about 103 dollars total. A month goes by and the package shows as delivered with a picture of a unit that isn't mine. Not even close.

I contacted them for a refund and they keep telling me I have to fill out this form they have. The form is really badly worded and it asks for a signature. I already gave them all the info they need. They say they can't look into it with the courier without that form. It doesn't make sense. So now I am stuck.
