My name was forged on paperwork for a home healthcare company

I got a text about some employment thing with Tempest Unlimited. I never signed any documents to become an employer for them or any home healthcare company. I was always the participant, the one with Medicaid. My parent caregiver at the time, Doreen Rhodes, forged my name countless times. She had me sign when I was really sick and just trying to get some rest. Now I'm seeing this and it's about $10,000. I don't know anything about this.
