Got a mail notice about my mortgage saying I have to pay a waiver fee

I got this mail that says it's time sensitive and it's from Union Saving Bank. It says they have an important matter about my property or mortgage and that they'll revoke a waiver in 7 days. At the bottom of the paper there's a waiver attached for $199.99.

The whole thing felt off. I never heard from them before about this. The paper had a phone number on it, (888) 788-4779, and another number for Spanish. I didn't call or pay anything. I just don't know why they'd send this.
