Medical supplies billed to my old insurance for almost 19 thousand dollars

I got two explanation of benefits in the mail from my old UHC insurance. The amounts were for almost $19,000 total. It says medical supplies were billed to my old plan.

I don’t know who sent the supplies or why. I didn’t order anything. The address on the EOB is 5118 Park Ave, Ste 323, Memphis, TN 38117 but I don’t know what that is.

No money was taken from me, but it’s scary seeing that much billed under my name. I just want it stopped.
