Bill for medical equipment I never got from Kaltrex Solutions

I got a bill that went through a really old insurance I had with UHC. Then I received a letter from them saying I'm going to owe over $4,500 to some company called Kaltrex Solutions in June 2026. The letter says it's for durable medical equipment. But I've never gotten any medical equipment from anyone. Not once.

This scam went through my old insurance and there's no doctor listed or even a description of what this is supposed to be. I have very good credit and I'm just so upset about this. I never ordered anything. I don't know how they got this through. Thank you for your help.

Medical supplies billed to my old insurance for almost 19 thousand dollars

I got two explanation of benefits in the mail from my old UHC insurance. The amounts were for almost $19,000 total. It says medical supplies were billed to my old plan.

I don’t know who sent the supplies or why. I didn’t order anything. The address on the EOB is 5118 Park Ave, Ste 323, Memphis, TN 38117 but I don’t know what that is.

No money was taken from me, but it’s scary seeing that much billed under my name. I just want it stopped.
