Another fake EOB for medical supplies I never ordered

I got another EOB from my insurance for over $11,000 for medical supplies. This is the third time this has happened. I don't know this company and I never ordered or used any medical supplies.

The address they have is in Memphis, TN. I have no idea who they are or how they got my insurance info. It just keeps happening and I'm tired of it.
