Medicare notice about catheters I never ordered

I got this letter in the mail. It says it’s a “Medicare Summary Notice for Part B” dated May 29, 2026. It says claims processed between Nov. 30, 2025 and May 29, 2026. But the claims listed are for April 22 to October 22, 2025. That doesn’t make sense.

It says someone ordered “300 Intermittent urinary catheter” and charged $2,250.00 each. For 5 months that’s $11,250.00 total. Medicare approved $2,250.00 each for 3 of the charges, that’s $6,750.00. They didn’t approve June and October charges. Says Medicare paid $5,292.00 and the max I may be billed is $1,350.00.

I have never ordered anything from this medical supply company. Their address is 1805 S Bellaire St Ste 480-01 in Denver, CO. I don’t know who they are. The notes say they sent a claim to UnitedHealth Group. This whole notice feels fraudulent to me. I didn’t ask for any of this.
