Medicare card in the mail with no company name on it

I got a card in the mail about Medicare open enrollment for when I turn 65. There is no company name anywhere on it. No return address except a PO Box in Boise, Idaho. No website or phone number or anything.

It looks like it is supposed to be from the government but they left off any corporate info on purpose. They are trying to make it look official when it is not. I have no idea who sent this or what they want. It just feels shady.
