Letter with a fake check and no company name about my property

I got a letter in the mail with what looks like a check for $195.00 saying it's an allocated waiver. It's signed by William H Keating. The letter has my name and my full postal address on it. There's a Record ID: 325583483.

It says it's about an important matter regarding the property at my street address, secured by my current lender. Then it says they can revoke the allocated waiver after 7 days from me getting this notice.

There's no company name anywhere on the letter. No website, no email, just a phone number (800) 823-7592 and an address in Ohio. I don't know who this is or what they want. Feels like a phishing thing.
