Mail saying I have a pending waiver check with no company name

I got two letters in the mail about a "pending approval" for something called lender info. There’s no company name on it anywhere. Just a phone number 1 800 380 5694 to call.

It says this is my final notice and I have to urgently contact them about a waiver check for $195.00 signed by some guy William H Keating. I don't know who that is. Never heard of this company or this check.

The address on the letter is Sacramento, CA 95813. I didn't lose any money but it felt weird getting this out of nowhere.
