EOB from BCBS showing equipment charges I never ordered

I got an EOB from BCBS the other day about two "equipment" charges totaling just under $5,000. It says they weren't covered by insurance and were submitted through Youngmans Equipment LLC. I have no idea what this is for.

I did a quick Google search on this company and it looks like they are known for doing stuff like this. I never ordered any equipment from them. Never heard of them before this letter showed up. Just feels wrong.
